Important information about clinic closure

Dear Patients,

Neo Health Clinic permanently closed on December 31, 2024 (the closure notice was emailed to all patients on November 15, 2024). The clinic’s appointment-booking assistants also stopped scheduling any appointments as of the closure date.

Due to the closure of the clinic, we worked very hard in the past few months to find new doctors for you. However, as of January 7, 2025, we have just learned that Radiant Medical Clinic (Dr. Irene Li & Dr. Brian Chang) is unable to open and provide medical services due to various issues. To assist you in finding a new healthcare provider, here are some suggestions:

1. Contact your friends and family to inquire if their family doctors are accepting new patients.
2. Speak with your local pharmacist, as they may know of family doctors or nurse practitioners in your area who are accepting new patients.
3. Check local newspapers or online community forums for clinic announcements about accepting new patients.
4. Call clinics in your area to ask if they are accepting new patients.
5. In the meantime, you can visit a local walk-in clinic for medical services.
6. For patients with OHIP, you can register with Health Care Connect through the following link: https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner to be placed on a waitlist for a family doctor or nurse practitioner.

Since the clinic's electronic medical records system has been closed, we are no longer able to provide copies of your medical records directly. All patient medical records have been transferred to a professional medical records storage company, Record Storage & Retrieval Services Inc. (RSRS), for safe storage in accordance with the regulations of the College of Nurses of Ontario (CNO). You can contact RSRS directly to obtain certified copies of your medical records. Please note that, according to RSRS policies, there may be a fee for obtaining or transferring your records.
